We are in-network for Medicare, Medicaid & many private insurances
Any child from birth through 18 years of age is eligible to receive Vaccines For Children (VFC) supplied vaccines if he/she meets certain eligibility criteria (Medicaid-eligible, uninsured, or underinsured). No VFC-eligible child is turned away for inability to pay.

You are responsible for knowing what facilities/services are in-network/covered by your insurance plan.
Need a copy of your immunization record?
Please stop of the Health Department at 2000 Countryside Drive during clinic hours to purchase copies of immunization records for $5.00 per copy. Cash, check and credit card are all accepted forms of payment. Please have one form of identification present at time of purchase.
*If under the age of 18, parent ID must be presented at time of purchase.
